Mind UR Health is a disability support services provider based in the Illawarra, New South Wales.
At Mind UR Health we provide a range of disability services. With the range of services we provide, we aim to make a difference in the lives of those living with a disability. The goal is to make each of our participant’s lives easier, fairer and more involved by participating within their own communities.
Mind UR Health is a service that was established with the sole aim of offering disability services that puts the participant as a priority in creating opportunities, promoting independence and progression in a safe and inclusive environment.
Mind UR Health complies with all NDIS Provider State and Commonwealth Government requirements for the delivery of quality and safe disability support services.

Support Coordination

Helping you to identify the best services and supports available.

Assisting you to coordinate your services and other supports.

Assisting you to resolve areas of conflict with providers.

Providing support, guidance and solutions when needed.

Providing training and support so that you can manage budgets and deal directly with service providers.

Monitoring your NDIS plan outcomes and providing reports back to the NDIA and assisting you to prepare for plan reviews.
Assistance With Travel and Transport
Making travel arrangements including provision of transport to access essential services and participation in the community activities.
Outside Participation in the Community
Supporting individual life skills including training in using public transport and developing skills for community, social and recreational participation.
Assistance to Access and Maintain Employment or Higher Education
Enabling participants to successfully obtain employment in the open or supported labour market.
Participation in Community, Social and Civic Activities
Supporting community activities including shopping, medical appointments, sporting and recreational events, social activities, including visiting and making new friends to confidence and social skills.
Accomodation/Tenancy Assistance
Support to guide, prompt or undertake activities to ensure appropriate accomodation is obtained/retained, assist with rental applications and tenancy obligations.
Assistance with Life Stage Transitions
Mentoring, peer support, individual skill development, including supporting decision making to make transitions in ways that meet goals and aspirations.

Specialised Support Coordination: Level 3

A higher level of support for participants whose situations are more complex and who need specialist supports

Specialist Support Coordination will assist participants to manage challenges in their supports environment and ensure a consistent service of delivery.

Address complex barriers

Crisis: Planning , prevention mitigation and management
Psychosocial Recovery Coaching

Develop recovering-enabling relationships for participants

Support participants in all aspects of the recovery planning

Coach participants in their personal capacity and recovery skills. These can include motivation and decision making.

Ensure all participant supports are recovery orientated.

Provide advocacy and engagement with the NDIS

Work with participants to implement recovery plan.
